Please call us at (503) 669-8081 to set up an evaluation time. Please inform us that this application was filled out online for quicker service. An evaluation may be waived for current DogTown students. Copies of the required vaccinations (DHLPP, Bordetella, and Rabies) must be mailed to: DogTown, 335 NE 8th Street, Gresham, OR 97030 or faxed to 503-669-8050. Dogs must also be current on flea treatment. There is a $15 fee for flea applications given, if necessary. Dogs over the age of six months must be spayed or neutered.

Hold Harmless Agreement

I agree that DogTown, LLC, its owners, employees, landlord/property owners and affiliates cannot be held responsible for any injury, illness, or damage which may occur to my dog or myself or any guest or family members.

I agree that my dog is in good health and free of any communicable diseases and is current on all required vaccinations.

I certify that my dog has no history of threatening and/or aggressive behavior and has never bitten another person or dog.

I understand and agree that any medical emergency that develops while my dog is under the care and supervision of DogTown, LLC, will be treated as deemed best by the sole discretion of DogTown, LLC and that our on-call Veterinarian may be consulted. I assume all financial responsibility for all expenses incurred.

I understand that I will be required to sign a paper copy of this agreement at the offices of DogTown, LLC before any services are rendered.
